CLASSROOM OBSERVATION ASSESSMENT

SENSORY PROFILE ASSESSMENTS

A sensory assessment can support you to explore and understand which sensory input in your environment is supportive to your emotional regulation and which sensory input is overwhelming. This is helpful information for divergent minds and bodies (chronic illness POTs, dysautonomia, PMOS, MCAS, EDS, hypermobility etc.).


The assessment & report provides recommendations and accomodations that support you/your child in their/your environments & in sensory processing.

  • Reduce Sensory overwhelm/shutdown/meltdowns

  • Prevent & manage burnout cycles

  • 1 x OT Session to briefly explore your sensory preferences

MAXI SENSORY REPORT

  • Access to an online sensory profile assessment

  • 2 x OT Sessions to explore your sensory preferences

  • 1 x OT Session to explore specific sensory barriers in a given environment (eg. work, school, uni, home) andproblem solve supports needed

  • Full report on your sensory profile, preferences, and suggestions for current sensory barriers

FUNCTIONAL CAPACITY ASSESSMENTS

  • This assessment and report inform the National Disability Insurance Scheme (NDIS) of what you/your child's current needs are from a physical, cognitive, and psychosocial framework to inform funding allocation on plans.

  • The process is as follows:


  1. A questionnaire is sent to you via email to gather necessary information about your current functional capacity and support needs, which is to be completed before attending your FCA appointment with an OT. 

  2. Attend a 45 min appointment with an OT to provide further context to the information already received. 

  3. Complete standardised assessments, email your NDIS goals and plan dates to the OT

  4. Provide relevant reports or documentation from other health professionals or learning institution about yours/your child's development.

  5. The OT collates this information into a report and then provides you with a feedback session on the report.


Diversum Health Group does NOT complete FCA for home modifications, specialist disability accomodation (SDA, SIL).

EXECUTIVE FUNCTIONING ASSESSMENTS

Executive functioning skills are located in the front part of your brain, and are an important set of skills that support you to make decisions on the steps required for planning, organising, and managing your activies and goals. 


If we have divergent minds and bodies, our executive functions can be a challenge to access in times of stress, overwhelm, pain, trauma, tiredness, low capacity, and can make it challenging to stay organised, remember details, control impulses, and transition between activities.

  • 2 x OT sessions to explore your excutive functioning strengths and challenges

  • 1 x OT session to explore specific strategies in a given environment (eg. school, work, uni, home, TAFE) 

  • Full report on your executive functioning strengths, challenges, strategies, and recommendations.
